The SAIL program – StoneRidge Assistance-in-Living – is designed for independent living residents who need a helping hand in their independent living apartment home. The SAIL program provides the support they need. Whether it’s support with showering or bathing, getting dressed, or taking medications as prescribed, our goal is to help them stay in their independent living apartment home for as long as possible. Care plans are customized to their needs and delivered with a personal touch from staff they already know.
What SAIL Provides
The SAIL program is as flexible as it is personalized. Care plans are developed to match each resident's specific situation and can be adjusted as needs evolve, delivered in the comfort of their own apartment.
- Assistance with activities of daily living: bathing, dressing, preparing meals, and mobility
- Medication management and administration
- Help scheduling medical
appointments alongside the resident by phone - Case management, in cooperation with
the resident's spouse or family - Discharge planning to and from the
hospital - Arrangements for non-medical services,
such as running errands, grocery shopping,
and laundry
